I acknowledge that responsibility for the loaned products shall transfer to me upon receipt, and I assume full responsibility for the handling and use of the products, including their use by third parties.

I acknowledge that I shall be held responsible for the actual cost of repair and/or replacement of the equipment which, during the loan period, is lost, stolen, or damaged for any reason whatsoever.
